In 1978, Dr. Robert H. Bloom and Dr. Jack R. Nizewitz opened their office in on Route 211 East in Middletown, NY. Dr. Nizewitz continued the practice and was joined by Dr. Gary J. Lake in the late 1980s.

The office moved location in the year 2000, and remains at its current address at 200 Midway Park Dr in Middletown to this day.

Dr. Nizewitz retired in 2018, and Dr. Lake continues to practice full-scope optometry today.

Dr. Debbie Ocampo joined the practice in 2021.
