Online Forms

At Adult and Pediatric Eyecare, we offer patient forms online so you can complete them in the convenience of your own home or office.​​​​​​​

While filling this out, you may want to have available a list of your current medications.

On the day of your appointment, don't forget to bring with you:

• Your vision insurance card if you have one
• Your medical insurance card, if any
  
  • If you are over 65, this includes your Medicare card and any secondary or supplemental insurance cards.
• Your list of medications
• Your current glasses
• If you are a contact lens wearer and will be having your contact lenses evaluated at your eye exam, please wear your contact lenses to your appointment
  • If you are a new patient to us, please bring in a proof of your current contact lens prescription. This can be a copy of the prescription, or a photo of the boxes from the lenses.
